Glossary and Terms:
Profession and Staff Services:
Coordinating Activities, Rites and Ceremonies (as described in the Funeral Act):
Covers the cost of a licensed Funeral Director to provide professional guidance and facilitate a proper plan for the family’s funeral wishes. May include but not limited to personal supervision by a Funeral Director for visitation or 24 hourtelephone answering support
The processing of all necessary legal and professional paperwork may include; Statement of Death, Medical Certificate of Death, Cremation and Cemetery documents, Clergy information, Proof of Death Documents, Statement of , Canada Pension Death Benefit, Cancellation Forms and/or other necessary forms.
Includes composition, editing, proofing, scanning and electronic transmission of photo(s) and inclusion on 24 hour taped funeral information line and funeral website.
Transportation of Remains and Vehicles:
Transfer from Place of Death/to Crematorium:
Includes all fees necessary for the professional transportation of the deceased. Includes but is not limited to all professional staffing, use of specialized vehicle, specialized equipment, transportation and cleaning. Please note an additional 100$ applies to transfers made outside of Monday to Friday from 9AM to 6PM.
Mileage outside Greater Sudbury
This fee applies to any transfer that is over 125 kilometers outside of Greater Sudbury. A rate of $2.00 per kilometer one way will be charged.
This fee applies to the use of the funeral hearse which is required to transport the casket during funeral processions or outside the funeral home.
This fee applies to the use of the limousine for use in funeral processions which can handle 7 comfortably.
This fee applies to the use of the lead car which is necessary for all processions which provide special traffic lights and is the vehicle in which the clergy is transported in.
This fee applies to the use of a service vehicle for various uses such as but not limited to transportation of flowers, delivery of paperwork or transportation of equipment.
Travelling Equipment and Set Up:
This fee includes the use of equipment such as Church Truck, Background and Catholic Equipment (Candles, Crucifix, Kneeler) for residential casket set ups, out of town church casket set ups or use of cemetery device for casket.
Preparation of Remains:
This fee applies when someone may wish the deceased to be viewed or at the wish of the purchaser. This may include, washing, setting features, and/or dressing the Remains, but does not include the embalming of the Remains. This must be done by a licensed funeral director.
The temporary restoration, disinfection and preservation of the body. This includes cosmetics, dressing and placing of the body in casket. Although it is not required by law, it is our suggestion to present an embalmed body for public viewing.
Fundamental Overhead Costs:
Covers basic operational cost of Funeral Home including; property taxes, heat, lights and other operational costs. This fee is approved by the Bereavement Authority of Ontario.
Facilities for Sheltering Remains
A room not available to the public set aside in the funeral home where your loved one is kept until paperwork is complete or period before visitation. This fee includes 72 hours of sheltering but may be increased depending on circumstances such as weekend and or holidays. After 72 hours we charge $50.00 per day.
Facility to identify Deceased:
Includes the use of the room and staff for one hour for the purpose of identification only. Does not include visitation.
Facilities for Visitation:
Includes the use of the facility for the purpose of visitation which includes but is not limited to room set up, use of religious equipment, floral set up, use of fellowship room for beverages (coffee, tea and juice included), cleaning of room for two hours and or use of television, DVD or multimedia equipment. Addition hours are available for a fee. See Below.
Staff Services for Visitation:
Includes the staffing necessary for the visitation for direction of guest, receptionist for donations, coat check, coffee room attendant. Addition hours are available for a fee. See Below.
Facilities for Ceremony:
The use of the chapel or visitation room for the funeral, memorial or prayer service. This fee applies to all locations and may include but not limited to use of religious, equipment, audio equipment and service recording.
Staff Services for Ceremony (onsite or traditional Church Service):
All professional staffing required for a funeral service. Applies to all Chapel or Church funerals. May include but not limited to funeral professionals, drivers for vehicles, picking up family, picking up clergy and or set up of church prior to service.
Facilities for Reception:
The use of the fellowship or visitation room for reception. Includes staffing, coffee, tea, juice, pickles, Finnish bread, cookies and clean up.
This fee applies to the food chosen by the purchaser. Typically includes a variety of sandwiches and or vegetable, fruit, dessert or cheese trays.
Staff Services for Ceremony (Memorial Service Offsite)
Funeral Director Present:
The delivery of all memorial service supplies. Includes delivery or Urn, memorial cards, register book, flowers and supervision of service from start to finish.
Receptionist for Donations:
This fee applies to the use of the receptionist to attend the service and take donations.
Delivery of Urn, Flowers, Paperwork:
The delivery ONLY of all memorial service supplies. Includes delivery or Urn, memorial cards, register book and flowers.
Organization, set-up and staff attendance of casket or ash burial.
Memorial Cards (Set Up Fee)
The creation, preparation, and publishing of memorial cards or bookmarks for families.
The actual printing of the memorial cards or bookmarks.
The book in which people sign when they enter the room. Also includes the write up of all donations, masses and floral tributes.
Thank You Cards:
Cards given to the family usually sent to people who send flowers, food or make donations.
A special container chosen by the family for the storage of cremated remains.
Outer Burial Container:
A specialized container used to house the casket or urn. Available at Lougheeds Limited include Wooden or Steel for Caskets and Plastic for Urns. Neither are required for a burial in the city of Sudbury.
An ornamented or non ornamented wooden or steel container where the physical remains are placed in for burial or cremation. A basic wooden cremation container is still required for cremation by law in Ontario.
Disbursements are items arranged for and paid for on behalf of the Recipient by the Provider. The Recipient is expected to pay for these items at the time the Contract is entered into. The following are some of the items often identified as disbursements
Municipal Death Registration Fee:
This is a fee charged by the local municipality to register the death. The fee is set by the municipality and may vary.
This is a fee charged by the Coroner for issuing a certificate that allows cremation to take place. The fee is set up by the Chief Coroner for Ontario.
This is a fee charged by the crematorium for the service of cremation.
This is a fee charged by the cemetery to provide burial of a casket or urn.
Clergy Honorarium or Church Fee:
This is an amount paid to the clergy for their services. In some cases, the amount is set by the clergy, while in others, it is an amount determined by you. Church fee is established by the Church for the use of their facility and people. May include, organist and actual Clergy. Most church fees include all services.
Musician/ Soloist/ Organist:
A person that comes to play, sing or both at a funeral service.
Newspaper or Radio Notices:
These are charges for death notices placed in newspapers. The newspaper sets the price.
Other Important Information:
The family may request the Provider to pay other disbursements, as they would find helpful. Such as reception fees offsite, additional musicians or flowers. Please not these must be agreed upon with the establishment and funeral director.
Payment, Financing and Funding
We are proud to offer payment, funding and financing options to our clients. Please contact us or advise us ASAP if financing is required to avoid additional fees.
Payment and Financing:
Unless otherwise specified, payment is due within 60 days of signing the Contract. Payment may be made by cash, cheque, debit or most major credit cards. Payments plans are also available to meet most budgets. Financing charges apply to payment plans. Beyond the allotted time, interest will be charged at a monthly rate of 1.5% (which is equivalent to 19.5% per annum).
If you are prepaying for services or supplies, pre-authorized direct payment is available. However, when pre-paying, the entire Contract amount must be paid within 12 months of signing the Prepaid Contract to receive the 10% pre-payment discount.
For your convenience, we offer a prepaying of services or supplies. When prepaying, you pay for the services and supplies today and the money will be used to pay for the services and supplies when they are delivered at the time of death.
You may pay the money directly to us and we will place the funds in trust, where it will accrue interest until the Contract is fulfilled (when the services and supplies are delivered) or the Contract is cancelled. Under Ontario Law, your Prepaid Contract would be guaranteed once it is fully executed. Should you have any questions about refunds or the removal of services it is best to contact us directly.
In acknowledgement of the PIPEDA Act, I have checked the Privacy waiver box to give the funeral home full access to myself for any and all information necessary in the funeral or auxiliary information, which the funeral home believes, would be to my benefit. Further I give permission to the funeral home to share any information with other organizations that it deems beneficial to me.
Possession of Cremated Remains
The Purchaser authorizes the Provider to take possession of the cremated remains of the recipient. The Provider will retain the cremated remains for no more than one year from the date of cremation. If the cremated remains are not claimed within one year, the Provider may inter the cremated remains in a cemetery, including in a common lot for which the cemetery is the interment rights holder and the Purchaser will pay all costs of the interment or subsequent storage charges. Possession of Cremated remains by the Provider holds a refundable deposit of $350.00.
